Privacy Policy & Terms

We do not share any client data with third parties. Your personal information is kept confidential and is not disclosed to any outside organizations, except as required by law or with your explicit consent.
 
Clinic Policies:
Clinic: Oregon Natural Medicine is not a primary care clinic and the physicians do not replace care or services provided through your PCP. It is recommended that all patients have a separate PCP.
 
Insurance: Oregon Natural Medicine is not credentialed with any insurance company and all visit fees are due at time of service.
 
Appointments: Oregon Natural Medicine has a 24 business hour cancellation policy. If an appointment is missed or canceled within 24 business hours, there will be a charge of $150 for the first instance. The full cost of the visit will be charged for any subsequent missed or cancelled visits.
 
Telephone: Oregon Natural Medicine does not maintain an after hours phone number and does not replace your primary care clinic or urgent care clinic. Phone messages will be returned during our normal business hours. Email: Messaging is available through our patient portal, please know that messaging to your physician is only intended for very brief clarifications of your treatment plans. We recommend only messaging through our HIPPA compliant medical record system. Any messages sent outside of this system are not guaranteed HIPPA compliant and therefore it is not recommended to be used for health or medical related concerns.
 
Texting/Messaging: Our messaging system is HIPAA complaint. By providing your contact information, you consent to receive text messages and other forms of electronic communication from us. These messages may include updates, reminders, and other relevant information. You can opt-out of these communications at any time by following the instructions provided in the message, by contacting us directly or simply by texting back ‘STOP’. Your contact information will be used solely for the purposes outlined in this consent and will be protected according to our privacy policy.
 
Supplements: We appreciate your supporting local business by purchasing your high quality nutritional, herbal and homeopathic supplements through Oregon Natural Medicine. Supplements are now purchased primarily through our online dispensary. We strive to keep our prices affordable. Please note that we are unable to refund any purchased product but will help you get refunded if issue is due to damaged products. Prescriptions & Refills: We require that patients with ongoing prescriptions have an in office or telehealth appointment at intervals recommended by their provider. If a patient is started on a new medication or new dosage, a specific return appointment timeframe will be outlined on the patient’s treatment plan. Prescriptions will not be refilled if a patient has not been seen within a 6 month timeframe or does not return at the previously recommended visit follow up.
Oregon Natural Medicine does not direct bill to insurance companies but may be able to provide you with necessary codes so that you may self submit with your insurance company.
 
Visit Fees: All visit costs are due at  he time of service. Cash, check, and credit cards are accepted (Visa, MasterCard, Discover and AMEX). HSA and FSA may be utilized for services (You are responsible for determining your eligibility).
 
Additional Fees: As a courtesy, Oregon Natural Medicine can fax labs, imaging or treatment plans from your last 12 months of care with a signed Release of Records. For records greater than 12 months, or if a patient’s full chart is requested, a fee of $25 will be charged for the transmission or printing.
 
Forms requiring review and completion by your physician (such as HSA, FSA, life insurance, or medical  eave forms) will be charged a $25 fee.
 
Appointment Frequency: Patients on any prescription medications or following specific protocols need to be seen a minimum of every six months or at intervals recommended by their provider. Patients not seen in more than 2 years, but requesting to return to the clinic as a patient, will be considered a new patient and will be charged the subsequent new patient fees.
 
Discharge: Patients may be discharged from the practice for any reason. Discharge may occur for failure to comply with office policies/agreements, failure to comply with treatment plans provided by your practitioner, missing or excessive cancellation of appointments, unpaid invoices over 90 days or due to any verbal abuse or threatening statements towards staff. If there are any problems with anything in our office, please calmly let us know and we will do our best to resolve it.
 
Emergency Care: We do not provide emergency medical care or after-hours treatment at Oregon Natural Medicine. If you are concerned that you may be experiencing a medical emergency, please call 911. If you are not experiencing a medical emergency, you may leave a voice message on our office phone 503-946-8700 and calls will be returned during normal visit hours.
 
Prepaid Packages: All appointment fees must be paid prior to appointment as outlined within the package. There are no refunds on prepaid packages. If an appointment is missed or canceled within 48 business hours of a patients’ scheduled appointment time, the prepaid package fees for that  appointment will be forfeited. Packages expire 1 year from purchase and any unused visits will be forfeited.
Hippa/Privacy Policy:
I understand that my medical record will be kept private. I understand that the clinical and support staff at Oregon Natural Medicine, LLC will have access to my medical record. Oregon Natural Medicine uses a HIPPA compliant electronic medical record system. I acknowledge that my information will never be disclosed to anyone with out my consent, except in the case where it is mandated by state law. I understand that I have the right to view my medical chart. I understand that I may request a copy of my medical chart by paying the set fee ($25) for printing/faxing services.
